End-of-Life

The end of life deserves as much beauty, care and respect as the beginning.

WHO WOULD BENEFIT FROM AN END-OF-LIFE SUPPORTER?

A person who has a life-limiting diagnosis, the caregiver, a family member, or someone who is involved with the care.

WHAT IS OFFERED
  • Honour the dying person's wishes

  • Emotional, practical support

  • Discussing and assisting to fill out advance directive forms

  • Navigate family conferences

  • Aid in the understanding of options relevant to client/families i.e. palliative care or hospice

  • Assist in accessing community resources i.e. homecare, PCOT team

  • Short respite for the caregiver 

  • Organize legacy work

  • Develop a plan for the death vigil

  • Conduct rituals or comforting practices

  • Education around the dying process and encouraging open discussion about death

  • Respect, privacy, and confidentiality

WHAT IS NOT OFFERED
  • Medical advice

  • Personal care

  • Administering of medications

  • Decision-making (assistance is offered in the form of information and resources so that the client and/or family can make a well-informed decision)
